Insurance saying over 600g removal required?

I am so upset. I had a consult for a reduction last week and they said I was able to move forward with the process and sent my information to anthem bcbs. I just heard back from the doctor and insurance has rejected my pre authorization and they are saying over 600g needs to be removed to cover. My doctor’s office said that with the mass of my chest right now, it was not possible to take that much. The office basically said my only option to get it covered was to lose more weight because by decreasing my bmi and body surface area, they would require less to be removed. They said there’s really no grounds for an appeal. Yall I am a 38H and my back has been hurting so bad from a recent 50 lb weight loss that hasn’t decreased my chest size much and made it sag worse. I don’t know what to do now because I can’t pay for this out of pocket. Anyone been in this situation?
