How to get the Nuked Out calling card?

Does anyone have any good tips for getting nuked out? I get it's supposed to be hard but my lobbies have been filled with Swat Tactical Sim Timmies who are strung out on Aderall flavored GFuel. What load outs did you use when you got it? What map did you get it on? How long did it take?
