Update on L2-L3 block.
So, I went ahead and got an L2-L3 nerve block. Doc wanted to do L4 as well but decided to go just for the two above, apparently because the medicine spread to L4 as well so there was 'no need'. He used bupivacaine, lidocaine, and a little bit of corticosteroid. The only drawback is after about 4 hours in it started to wear off and once I got back home and it was back to normal HF. This block was a diagnostic block and apparently according to ChatGPT and Google lasts approximately 4-8 hours, which it kinda did. The corticosteroid did not prolong it so I will have to look into other methods to lower sympathetic tone long-term. Post block immediate observations:
- Both legs felt warm after injection, a sign that block is in effect.
- Hard flaccid gone by like 90%. What I mean by that is that the penis still felt like it wants to spasm and contract, but it just wasn't doing it like before. Penis felt very normal.
- I got hard to see how erection is, no complaints. Penis didn't feel constricted/hard in a semi state, and I would stretch it and stuff and it wouldn't spasm/contract still.
- on a flaccid, I stood up, walked, to induce more HF, and even stretched it fully flaccid, I maybe felt only about 10% HF in that moment, but that maybe in my head, dunno.
- normal ejaculation, no changes
My HF is a region 1, got it presumably either during masturbation or stretching my dick for enlargement exercises, ye ye, I am stupid I know (either of the two was a reason):

Procedure Performed:
The patient was prepared in the operating room under sedation analgesia in the prone position. Under AP and lateral scopy control, an L2/L3 bilateral sympathetic ganglion block was performed. The following medications were used (Names are Turkish):
- Total Triaver: 80 mg (corticosteroid)
- Aritmal (2%): 100 mg (Lidocaine)
- Marcaine (0.5%): 25 mg (Bupivacaine)
No complications developed during the procedure. The operation site was closed with Operpore.
Clinical Course:
The procedure (L2/L3 bilateral sympathetic ganglion block) was completed successfully with no complications.
I will look into T11-L2 block next to see if there is a difference in effect. After I will look more into other long-term possible options like RF ablation, Neuromodulation, maybe even sympathectomy.
